Dress Giveaway Application

Welcome to our dress giveaway application!

Eligibility requirements for our giveaway:

• Applicants must be currently enrolled in high school as a junior or senior

• Applicants must maintain a GPA of 2.0 or higher

• Applicants must be able to demonstrate financial need

​

This application consists of:

• Personal info including your name, contact information, address, and school information

• A written paragraph explaining why going to prom is important to you, and why you are applying for our giveaway

• A media release consent form

  • The signed photo release form and gives Marias Closet permission to use recipient’s photographs in marketing and promotional materials. (If under the age of 18, a parent or authorized guardian must sign this document on applicant’s behalf)

You will not be able to save progress on your application, so before you start, make sure you have enough time to complete the whole process. The whole form not including the essay portion takes about 10 -15 minutes to complete. We suggest that, if you do not have time to write the full essay in one sitting, you write it in another location (like Google Docs, Microsoft Word, or even your notes app) and copy/paste or upload it when you fill out this form.
